Overview

This unit describes the performance outcomes, skills and knowledge required to plan, monitor and review case management.
Workers at this level work within organisational policies and procedures and may work autonomously or in consultation with supervisor or team leader.This unit applies to work in a range of health and community services contexts.

Assessment

For Melbourne campuses

Assessment tasks will be designed to reinforce & extend knowledge and skill competence within set & controlled parameters in accordance with each units learning outcomes & performance criteria requirements, including the setting of work based practical application tasks designed to provide evidence of competence outcomes, within periodic and scheduled timelines.

Students will be expected to demonstrate the following required skills:

  • develop, facilitate and review all aspects of case management for three people
  • performed the activities outlined in the performance criteria of this unit during a period of at least 100 hours of work within a workplace. These hours cannot be completed concurrently with hours specified in any other unit of competency.

Required reading

The qualified trainer and assessor will provide teaching and learning materials as required in the form of workbooks produced by Victoria University and/or via the Victoria University e-learning system.
